If your proposed research study requires participant recruitment/consent, you must follow the steps below to ensure compliance with British Columbia's Freedom of Information and Protection of Privacy Act (FIPPA) regarding the collection, use, linkage, and/or disclosure of personal information for research and evaluation purposes.
Consult your Research Ethics Board (REB) for the appropriate consent form template.
The BC Ministry of Health now requires a Consent Form Attestation to be completed: please see BC MoH Letter to Researchers. Refer to page two of the Consent Form Attestation and ensure that your consent form meets all the requirements listed.
Obtain approval for your consent form from your REB.
At the time of your DAR submission, you will need to:
- Provide a copy of your consent form that was approved by your REB.
- Ensure that your DAR includes the information requested on page 2 of the Consent Form Attestation.
- Provide a complete and signed copy of the Consent Form Attestation.
